Qusel (quetiapine)

Description

Qusel of Hilton Pharma is a psychotropic agent of dibenzothiazepine derivative class.

Mode of Action

Qusel act by antagonizing serotonin 5HT1A, 5HT2, dopanine D1 and D2, histamine H1 and adrenergic (alpha-1 & alpha-2) receptor, qusel exhibits minimal affinity for cholinergic, muscarinic and benzodiazepine receptors.

Indications

1. Indicated Schizophrenia.
2. Indicated in acute manic episodes associated with bipolar 1 disorder.

Contraindications

It is contraindicated in patients with known hypersensitivity to Quetiapine or any other ingredient of the product.

Use in pregnancy

Since adequate and well-controlled studies are not available to establish the safety of qusel (Quetiapine) in pregnant woman, qusel should be used during pregnancy only if the potential benefit to the patient outweighs the risk to the fetus.

Use in nursing mother

Though it is not know if qusel (Quetiapine) is excreted in breast milk, qusel should be given to nursing mothers only when potential benefit to the mother outweighs risk to the child.

Use in children

There is no sufficient data to establish the efficacy and safety of qusel in children.

Use in old age & debilitated patient

In old age and debilitated patients, qusel should be up titrated slowly & target dose should be set at lower level.

Side effects

Following side effects have been found to be associated with usage of qusel (frequent: seen in at least 1/100 patients, infrequent: seen in 1/100 to 1/1000 patients).

Nervous system
Frequent: qusel cause hypertonia, dysarthria;
Infrequent: qusel cause abnormal dreams, dyskinesia, thinking abnormal, tradive dyskinesia, vertigo, involuntary, confusion, amnesia, psychosis, hallucination, libido increased, urinary retention, incoordination, paranoid reaction, abnormal gait, myoclonus, delusion, manic reaction, apathy, ataxia, depersonalization, stupor, bruxium, catatonic reaction hemiplegia.

Body as a whole
Frequent: qusel cause flu syndrome;
Infrequent: qusel cause neck pain, pelvic pain, suicide attempt, malasia, photosensitivity reaction, chills, face adema, moniliasis.

Digestive system
Frequent: qusel cause anorexia;
Infrequent: qusel increase salivation, increase appetite, gamma glutamyl transpeptidase increase, gingivitis, dysphagia, flatulence, gastroenteritis, hemorrhoids, stomatitis, thirst, tooth caries, fecal incontinence, gestroesophageal reflux, bum hemorrhage, mouth ulceration, rectal hemorrhage, tongue edema.

Cardiovascular system
Frequent: palpitation
Infrequent: Qusel cause vasodilatation, QT interval prolonged, migraine, bradycardia, cerebral ischemia, irregular pulse, T wave abnormality, bundle branch block, cerebrovascular accident, deep thrombophlebitis, T wave inversion.

Respiratory system
Frequent: pharyngitis, rhinitis, cough increased, dyspnea.
Infrequent: pneumonia, epistaxis, asthma.

Metabolic and nutrional system
Frequent: peripheral edema.
Infrequent: weight loss, alkaline phosphatase increased, hyperlipemia, alcohol intolerance, dehydration, hyperglycemia, creatinine increased, hypoglycemia.

Skin and appendages system
Frequent: Sweating.
Infrequent: pruritus, acne, eczema, contact dermatitis, maculopapular rash, seborrhea.

Urogenital system
Infrequent: dysmenorrheal, vaginitis, urinary incontinence, metrorrhagia, importence, Dysuria, vaginalmoniliasis, abnormal, ejaculation, cystitis, urinary incontinence, metrorrhagia, female lactation, leucorrhea, vaginal hemorrhage, vulvovaginititis, orchitis.

Special senses
Infrequent: conjunctivitis, abnormal vision, dries eyes, tinnitus, taste perversion, belpharitis, and eye pain.

Musculoskeletal system
Infrequent: pathological fracture, myasthenia, twitching, arthralgia, arthritis, leg cramps, and bone pain.

Hemic and Lymphatic system
Frequent: leucopenia.
Infrequent: leukocytosis, anemia, ecchymosis, eosinophilla, hypochromic amemia, lymphadenopathy, cyanosis.

Endocrine system
Infrequent: hypothyroidism, diabetes mellitus.

Drug interactions

Qusel should be cautiously co-prescribed with other centrally acting drugs. Since quseL (Quetiapine) may cause hypotension, it may potentiate the effects of antihypertensive agents.

Qusel (Quetiapine) may antagonize the effects of levodopa and dopamine agonists. Since co-administration of Quetiapine (qusel) and hepatic enzyme inducer e.g., phenytoni, carbamazepine, barbiturates, rifampin & glucocorticoids, increases the mean oral clearance of QUSEL, higher dose of qusel may be required to be given in such patients to ensure optimum efficacy in schizophrenia.

Day 3rd 300mg/day
Day 4th 400mg/day

Maintenance dose
Dose may adjust by up titrating to the maximum of qusel 800mg/day.

Qusel 25mg each tablet contains Quetiapine 25mg.
Qusel 100mg each tablet contains Quetiapine 100mg.
Qusel 200mg each tablet contains Quetiapine 200mg.
